  • Long-Acting Injectables: Improving Team Member Engagement Across Four NYC HIV Clinics
    The process of initiating patients on Antiretroviral Long-Acting Injectables (LAI) Cabotegravir/Rilpivirine (Cabenuva) and Cabotegravir (Apretude) was unclear and complicated for the Institute for Advanced Medicine’s (IAM) four HIV clinics. The team used the Plan, Do, Study, Act (PDSA) Model to improve the Long-Acting Injectable program. It required a collaborative effort between the LAI workgroup, nursing and medical leadership, Mount Sinai pharmacists, and benefits specialists.
  • Implementing a Tailored Approach to Provide Affirming HIV Care to LGBTQIA+ Youth
  • Leveraging Medicaid and Hospital BPAs to Link People With HIV to Care
    Data to Care (D2C) is a CDC-endorsed HIV Care program. The newest additions to the D2C “toolbox” of interventions include: using Medicaid Prescription refill data and Hospital Best Practice Alerts (BPA). The D2C Medicaid project uses prescription pickup information from claims to reduce antiretroviral therapy (ART) interruption among PWH, leading to improved retention in care. The new BPA D2C program is a form of provider D2C. This program will leverage a BPA system that a large hospital group recently implemented. PWH who are flagged for follow up via the BPA will be sent to state linkage workers. Those state workers will then assist with re-linkage to care and any other services needed.
